Title IX Complaint Form:
The purpose of this Title IX complaint form is to gather the essential facts as to the alleged actions to allow for the prompt, efficient, and equitable resolution to complaints based on sex discrimination, including complaints of sex-based harassment, or retaliation in violation of Title IX of the Education Amendments of 1972 (“Title IX”). This form applies only to complaints alleging sex discrimination or retaliation prohibited by Title IX.
By submitting this form online at the link below, the building based administrator and the Director of Special Education will receive the complaint.

Report of Discrimination, Bullying, Harassment, and/or Hazing:
The purpose of this report of discrimination, bullying, harassment, and/or hazing form is to gather the essential facts as to the alleged actions to allow for the prompt and equitable resolution to complaints based on bullying, harassment, and/or hazing. This form applies only to complaints alleging discrimination, bullying, harassment, and/or hazing and does not apply to allegations of sex discrimination, including sex-based harassment, prohibited under Board Policy 411.3/511.3.
